I hereby give Village of Hope America consent to record, videotape and photograph my image and/or voice to be used in the following ways:

• Company intranet accessible by employees only
• Company website accessible to the public
• Employee newsletter
• Client/customer newsletter
• Printed annual report
• Posts on Social Media, and
• Any other application deemed appropriate by VOHA staff

I understand that no special compensation will be provided to me for use of my image and that I may not be informed in advance of the specific use of my image.

By submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in immediate dismissal. By checking “I Agree” below, I affirm that I will abide by the client confidentiality protocols, and COVID safety protocols, as outlined below.

Client Confidentiality Protocol:

I shall respect the privacy of the people we serve and hold in confidence all information obtained in the course of volunteer services, whether that information is obtained directly or indirectly. Therefore, I will not disclose confidences to anyone except:

1. as mandated by law;

2. to prevent a clear and immediate danger to a person or persons;

3. where I am a defendant or witness in a civil, criminal or disciplinary action arising from the contract;

4. if there is a waiver previously obtained in writing, and then such information may be revealed only in accordance with the terms of the waiver.

I shall be responsible for storage or disposal of professional records in ways that maintain confidentiality.

I shall possess a professional attitude which upholds confidentiality toward the people we serve, colleagues, applicants, and any sensitive situations arising within the agency, and will maintain this professional conduct upon termination.

I understand that violation of this confidentiality statement may be grounds for immediate dismissal.

COVID Safety Protocol: 

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Village of Hope America has put in place preventative measures to reduce the spread of COVID-19; however, Village of Hope America cannot guarantee that you will not become infected with COVID-19. Further, attending a Village of Hope America program could increase your risk of contracting COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk I may be exposed to or infected by COVID-19 by attending the Village of Hope America program and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I also agree to comply with all federal, state and local mandates, as well as any policies of Village of Hope America, relating to social distancing, hygiene, cleaning and disinfection and any required health screenings. I also agree to immediately notify the Volunteer Manager if I experience any of the foregoing while on program. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Village of Hope America, its employees, agents, and representatives, whether a COVID- 19 infection occurs before, during, or after participation in any Village of Hope America program.

• Prior to starting your next onsite engagement at VOHA, volunteers must be fully vaccinated against COVID-19, and wear a mask at all times when onsite.

• VOHA is unable to accommodate requests for medical or religious exemptions for volunteers.

• Even if vaccinated, if you are experiencing any COVID 19 symptoms, or have been in close contact with someone who was positive for COVID 19 in the last 14 days you may not volunteer in person at VOHA until symptoms resolve.

• While onsite, volunteers must comply with all preventative measures put in place by VOHA (link to COVID-19 Policies and Practices for Volunteers document) and wear a face covering at all times when working. 

• Face coverings must:
  1. completely cover the nose and mouth
  2. be made with two or more layers of a breathable fabric that is tightly woven
  3. be secured to the head with ties, ear loops, or elastic bands that go behind the head
  4. fit snugly over the nose, mouth, and chin with no large gaps on the outside of the face
  5. be a solid piece of material without slits, exhalation valves, visible holes, punctures, or other openings.
